If you work with teenagers, run your church’s social media accounts, or in any way are using photos of people online, you may be leaving yourself open for a lawsuit. Posting photos of people, especially teenagers who are not of legal consenting age, is a very tricky business and if your business or ministry works with this age group, you need to have the right forms and signatures in place to ensure you are covered.
The form provided below is intended for the parents or guardians to sign once a year and include the names of everyone in their family. It does also give them the option to opt-out of the service if necessary and give your church or ministry one document for up to seven people at a time. Honestly, you should simply have everyone in your congregation or in your circle of influence to fill this out as a just-in-case mindset.
